Hallucination Risk Index is a proposed score for estimating how likely AI systems and AI-driven search features are to misstate facts from your pages. It matters because AI citations can distort pricing, medical claims, product specs, and brand attribution long before a human ever clicks through.
Hallucination Risk Index (HRI) is an internal scoring model that estimates how easily AI systems misquote, misattribute, or invent details from your content. For SEO teams, the value is practical: it helps you identify URLs most likely to be mangled in ChatGPT, Perplexity, and Google AI-generated search experiences before the damage shows up in support tickets or lost assisted conversions.
Important caveat. HRI is not a standard metric from Google, Ahrefs, Semrush, Moz, or Surfer SEO. You define it yourself. That means the score can be useful for prioritization, but the number is only as good as the prompts, sampling, and QA process behind it.
Most teams score HRI on a 0-100 scale. Lower is better. A sensible model usually combines a few signals:
If you want a benchmark, many teams treat under 30 as low risk, 30-70 as moderate, and 70+ as high risk. Those thresholds are operational, not universal truth.
Use HRI like a triage layer, not a vanity KPI. Pull candidate URLs from Google Search Console based on impressions for queries already triggering AI Overviews, then crawl them in Screaming Frog to find inconsistent titles, outdated copy blocks, missing schema, and duplicate fact patterns. Cross-check authority and citation gaps with Ahrefs or Semrush. If a page has high impressions, weak referring-domain support, and contradictory claims across the site, it is a cleanup candidate.
Good HRI remediation is boring. Tighten fact tables. Standardize pricing language. Add named sources. Reduce version drift between blog, docs, and landing pages. In regulated spaces, this matters more than clever copy.
Google's John Mueller confirmed in 2025 that structured data helps search engines understand content, but it does not guarantee how AI systems will summarize or cite that content.
This is the part people skip. AI outputs are unstable. The same prompt can produce different answers by location, account state, model version, and retrieval timing. So an HRI score can look precise while hiding noisy inputs. Also, not every hallucination is caused by your page. Sometimes the model is pulling from stale third-party sources, forum posts, or its own bad synthesis.
Bottom line: HRI is useful if you treat it as a repeatable internal risk model tied to real pages, real prompts, and real business impact. It is not a universal SEO metric. It is a QA system for the AI citation era.
